Here is everything you need to know before we can send you an offer letter.


1) Certifications - CPR & First Aid & Article 9.

All three certifications are required. If you don't have one or all, you can schedule to get trained. In order to move to the next step, it is required that you get certified in each. We will reimburse you for the training once you complete 40 hours of service.

2) Fingerprint Clearance Card (or the application you submitted to receive it.)

3) Notarized Criminal History

4) Two Forms of Identification

Accepted documents include US Passport, Driver's License, Birth Certificate, or Social Security Card.

5) Proof of Valid Transportation & Insurance

Each of the following is required:

  • ADOT Motor Vehicle Report (you can get it here)

  • Copy of your vehicle's current registration

  • Proof of car insurance (a copy)

6) References

You must submit three references and include their name, relationship, number of years you've known them, and an email address and phone number.
